Typical Coverage (Quick Reference):
- UHC/UMR: Usually unlimited for metabolic (prediabetes, cholesterol) and weight-related issues (obesity, overweight). No co-pay.
- Aetna: Up to 26 visits/year for general nutrition, weight loss, and metabolic health. No co-pay.
- Blue Shield: Often covers obesity, diabetes, overweight, and cholesterol management (unlimited).
- Anthem Blue Cross: Typically unlimited for most plans. Most have no co-pays.
- Hill Physicians | Sutter: Generally unlimited for most nutrition-related issues.
- CCHP | Alameda Alliance: Typically ~7 visits/year.
- Medicare: Coverage for Stage 3-5 CKD or T2 Diabetes (3 hrs first year, 2 hrs yearly thereafter).

Step 1: Gather Your Information
- Locate your insurance card.
- Identify the Member Services phone number
(usually on the back).

Step 2: Make a Quick Call
Call your Member Services and clearly ask:
“Hi, I’d like to verify coverage for Medical Nutrition Therapy (nutrition counseling). Please confirm if CPT codes 97802 and 97803 are covered.”
To follow up, (Blue Shield, Anthem ONLY) ask this:
“Do I have a deductible or copay for these visits?”
