What is the cost of nutritional counseling if you do not participate with my insurance?

In the event that you carry insurance where we are considered out of network, you will need to pay for your nutritional counseling session at the time of service.

I am happy to provide you with a “Superbill” and/or an itemized invoice you can submit to your insurance company for reimbursement. Reimbursement is based on your policy’s standards. Please call your insurance company’s member services number to verify how reimbursement works under your current insurance policy.

Please note if you do have insurance that considers me as out-of-network, I cannot submit directly on your behalf – I can only supply you with the appropriate paperwork. Therefore, payment is due at the time of service directly payable to John Canlas, RD.

Initial visits are $190.00/hour and each follow-up visit is $95.00/30 minutes. I accept debit cards and all major credit cards.

I live far away. Can I use my insurance for telehealth services with you?

Right now due to the pandemic, most insurances are covering telehealth services. Therefore, when you schedule your visit, you will be directed towards the next steps to setting up properly for your telehealth session.

Do you accept cash for nutritional counseling sessions?

Yes. Initial visits cost is $190 per visit (up to 60 minutes), and follow-ups are $95 for up to 30 minutes per visit. All sessions must be paid before the visit.

Are nutritional counseling sessions held virtually or in-person?

Both! In-person sessions are Tuesdays to Fridays only at my Concord, CA office. This location enables me to provide local nutritional counseling services to clients from Concord, Fairfield, Oakland, San Francisco, Pittsburgh, Walnut Creek, Brentwood, and many of the surrounding areas. You will have an option to book either a virtual or in-person appointment in my app called Practice Better. Please arrive at least 5 minutes early for in-person nutritional counseling sessions.

What do I need to do to use my insurance to work with you?

Step 1: You will need to verify if I am in network with your insurance and if you have any co-pay, co-pay insurance, etc. Please refer to this guide HERE on what to ask your insurance company. The phone number is usually the 1-800 number on the back of your card.

Step 2: You may ask your physician to refer you to my services by faxing (925-887-0342) the referral to me or you can bring this document HERE to your physician and have them sign it. Then, you can email it to me or fax it to me. HMO plans like Hills Physicians need to be pre-authorized FIRST before being approved. Please note that a referral is NOT a guarantee of coverage.
